Treatment consists in injecting botulinum toxin into certain muscle groups which are selected based on the type and depth of the patient’s expression lines.  Botulinum toxin is a protein that has been used for many years in various medical fields to reach diverse objectives and has proven to be quite safe. The injection is administered in one session that lasts approximately fifteen minutes. Topical anesthetic cream can be applied before the injection to minimize discomfort. The patient has a check-up after two weeks and again after six months.

Advice for the home:

·It is not advisable to do intense physical activity directly after the treatment.

·The patient must not go to sleep immediately following the procedure.

·It is not advisable to rub injected areas.

Peyronie’s disease is characterized by the hardening of fibrous plaques in the penis’ soft tissue producing curvature or slight turning to the side where the hard “plaques” are found. Sometimes a minor case of the disease occurs and can spontaneously disappear in 6-18 months. The disease is not malignant but it can produce pain and hinder a man’s ability to enjoy and perform a full range of sexual activities. In severe cases the plaques become even harder and produce pain in the penis. Significant curvature can hinder the man’s ability to have sexual intercourse (coitus). Plaques that appear on the upper and lower part of the penis can lead to shortening of the penis and a reduction in its size. One study found that Peyronie’s disease affects 1% of men. Most commonly it occurs at middle age (40-50 years old) although it can also affect young and elderly men. It is not linked to a specific age; it is believed to be a hereditary disease based on observations of multiple cases present in the members of one family.

Penis curvature symptoms

Generally the patient schedules a consultation based on pain or difficulty maintaining sexual relations. Occasionally the disease can cause sexual impotence due to the hardening of the affected area. Although the causes of the disease are not clear, some researchers believe that it could be caused by trauma to the penis that produces internal bleeding (corpus cavernosum); this would produce inflammation that would heal slowly. If more bleeding occurs before the first instance heals, inflammation could become chronic leading to permanent hardening of tissues (fibrosis) and even leading to calcification in areas of the penis.

Penis curvature treatment (non-surgical/surgical)

Occasionally, and without knowing exactly why, the plaques disappear spontaneously; because of this waiting between one and two years before turning to surgical solutions for treatment is recommended. Non-surgical treatments for this disease have not proven to be effective. Some doctors recommend the use of vitamin E or a vitamin B complex taken orally. Some researchers have tried injecting chemical agents, such as collagenase, corticosteroid, dimethylsulfoxide, etc., directly into the plaque without seeing encouraging results. Treating the penis with a low dose of radiotherapy has also been tested without favorable results, but to the contrary, has produced undesirable effects. There are two surgical treatments: complete removal of the plaque replacing it with skin or artificial material, or the removal of a band of tissue opposite the plaque without acting on the plaque itself. The later procedure is known as “Nesbit,” in honor of the surgeon who discovered it, and is currently the most frequently used surgical procedure. The patient must keep in mind that this procedure can shorten the penis by up to 2 centimeters (0.8 inches). In cases in which the patient suffers from sexual impotence a prosthetic penis can be implanted (cylinders of silicone that are placed in the interior of the penis, one on each side).

Occasionally penises of normal length have a diameter of less than 7 centimeters (2.8); and are considered very thin penises. In these cases it is possible to undergo a penis widening procedure, which is realized using fat grafts taken from other parts of the patient’s body.

Through a small suprapubic incision, nearby fat is obtained, this fat is then placed around the penis increasing the penis’ width.

Because of the surgical technique used and to improve the results of the procedure, circumcision is also necessary.

Postoperative: Penis Widening

The client must abstain from all sexual relations for approximately one month. In all cases, adequate rest that allows for the tissue to close and for movement of the penis to be avoided, is of great importance.

The appearance of edema (swelling) after surgery is quite frequent and lasts for varying amounts of time depending on the patient; because of this, it is very important to follow the recommendations indicated after the procedure.

It is not a painful procedure and after 12-24 hours the patient can be discharged, barring complications. The procedure is carried out using general or local-regional anesthesia, depending on the patient.

Penis size is a topic that concerns both men and women. When a penis is small, it is possible for women to “not feel anything” and for men to feel anxious when faced with not satisfying their partner when they do not grow to the desired length. An erect penis that measures less than 12 centimeters (4.7 inches) in length can be considered small; approximately 3% of men fit into this category. A flaccid penis that measures less than 6 centimeters (2.4 inches) in length can be considered small. However, it is important to keep in mind that the size of a flaccid penis does not have much to do with the size that it can reach when erect. Most commonly, small penises grow more than big penises to reach the same size when erect. Because of this, seeing a small flaccid penis does not say much about its size when it becomes erect and increases in size and length.

Is it possible to achieve a significant increase in penis length?

Yes. Physical and surgical methods exist to achieve this. Physical methods consist in penis extenders, which, when used correctly and according to the age of the patient, allow for an extension of 2 cm (0.8 inches). Depending on the anatomy of the penis, surgical methods also allow for an extension of between 2 and 4 cm (0.8 and 1.6 inches).

What are the indications for penis enlargement surgery?

1. Having a micropenis.
2. Traumatic amputation of part of the length of the penis.
3. Some congenital or acquired urological diseases.
4. The desire to increase low self-esteem.

What methods do our centers use for penis enlargement?

Surgical penis enlargement consists in cutting the suspensory ligament and lateral ligaments. This procedure brings more of the penis outside of the body (approximately a third to half of the penis is naturally inside the body) achieving visible penis enlargement, but only when flaccid, without determined changes in the erect penis. It is not a painful procedure and after 12-24 hours the patient can be discharged, barring complications. Avoiding sexual relations for 4 weeks after the procedure is recommended. The surgery is performed using local-regional anesthesia. Surgical penis enlargement has faster results than physical extenders. Occasionally, penis widening or thickening can be realized at the same time as lengthening. Surgery can also be accompanied by physical stretching exercises or by the use of mechanical extenders with the objective of consolidating the effects and if possible, increasing the effects achieved with surgery.
